Important Notice About This FormThis private form is for adults invited by BRLS’s RLB 2026 Program and Volunteer Coordinator (the “Coordinator”) or another representative expressly authorized by BRLS.
Submission completes the invited person’s enrollment and conditional acceptance into the BRLS volunteer program. It does not, by itself, authorize an activity. Before assisting, the volunteer must accept a specific activity requested and authorized by the Coordinator or another expressly authorized BRLS representative.
This form applies prospectively from the time it is submitted. It supersedes any earlier BRLS volunteer enrollment or acknowledgment only for activities occurring after this submission.
